In Rhode Island, beginning at age 14, schools are required to invite students to participate in their annual Individual Education Program (IEP) planning meetings, including transition. This website was developed for students, families, and staff to help plan for transition from high school to employment, further education, training and independent living. Transition services are a coordinated set of activities designed to improve the academic and functional achievement of a student with a disability to facilitate the movement from school to post-secondary settings, including post-secondary education, vocational education, integrated employment, continuing and adult education, adult services, independent living, and community participation.
